Premature ventricular contractions, commonly abbreviated as PVCs, are abnormal heartbeats that originate from the ventricles, the lower chambers of the heart, before the normal electrical impulse arrives from the sinoatrial node and conducting system. These extra beats occur earlier than expected in the cardiac cycle and disrupt the normal rhythm of the heart. In horses, PVCs represent one of the most commonly encountered ventricular arrhythmias and can range from incidental findings of no clinical significance to indicators of serious underlying cardiac disease that poses risk for sudden death.
The prevalence of PVCs in the general horse population is difficult to determine because many horses with occasional ventricular ectopy never undergo electrocardiographic evaluation. Studies of athletic horses undergoing cardiac screening suggest that PVCs occur in a small but meaningful percentage of the population, with detection rates varying based on the duration of monitoring and the clinical context. Single or rare PVCs may be found in otherwise healthy horses and may represent normal variants, while frequent or complex PVCs are more likely to indicate pathology. Performance horses receive the most thorough cardiac evaluation, potentially leading to detection bias in these populations.
The clinical impact of PVCs in horses depends on their frequency, complexity, and underlying cause. Rare, isolated PVCs in a horse with no other cardiac abnormalities often have no clinical significance and do not affect performance or safety. In contrast, frequent PVCs, those occurring in runs or pairs, or those arising from multiple ventricular foci raise concerns about electrical instability that could degenerate into life-threatening ventricular tachycardia or fibrillation. The risk of sudden cardiac death during exercise is the primary concern with significant ventricular arrhythmias, making accurate characterization essential for horses in athletic use.
With appropriate evaluation to determine the underlying cause and significance of PVCs, many horses can be managed successfully. Those with incidental findings and no structural heart disease may continue their activities with periodic monitoring. Horses with PVCs secondary to treatable conditions often show resolution once the underlying problem is addressed. Those with significant structural heart disease require careful evaluation regarding their suitability for athletic use. Understanding the spectrum of significance that PVCs can represent helps veterinarians and owners make informed decisions about management and exercise recommendations.
