This membrane contains blood vessels that nourish the bone, nerve fibers that sense pain, and osteoprogenitor cells capable of forming new bone tissue. When subjected to excessive or repetitive stress, the periosteum becomes inflamed and painful, often triggering new bone formation as a healing response. In horses, periostitis most commonly affects the cannon bones of young horses in training, where it is known as bucked shins, but can occur on virtually any bone subjected to trauma, concussion, or strain.
Periostitis affects horses of all breeds and disciplines but demonstrates highest prevalence in young Thoroughbreds and Standardbreds entering race training. The condition commonly develops when immature bone is subjected to the repetitive concussive forces of galloping before adequate skeletal adaptation occurs. Sport horses, particularly those beginning jumping careers, may develop periostitis affecting various bones of the distal limb. Any horse can develop localized periostitis from direct trauma, kicks, or interference injuries, making this condition relevant across the entire equine population regardless of intended use.
The impact of periostitis on equine health and performance ranges from temporary training setbacks to career-threatening complications if not properly managed. Simple periostitis typically responds well to rest and appropriate management, with most horses returning to full training within weeks to months. However, continued training through pain can lead to stress fractures, severe dorsal metacarpal disease, or chronic bone changes that permanently alter limb conformation. Early recognition and appropriate response to periosteal inflammation typically results in excellent outcomes, while ignoring early signs invites more serious injury.
Early detection of periostitis significantly influences treatment success and long-term outcomes. The periosteum is richly innervated, making early periostitis quite painful and typically causing recognizable lameness before serious bone damage occurs. Handlers and trainers who recognize the signs of developing periostitis can implement rest protocols that prevent progression to more severe injury.
