Preputial injuries often accompany penile trauma when the traumatic event affects both structures, and management must address both areas. Hematoma formation within the corpus cavernosum or subcutaneous tissues represents a common consequence of blunt trauma that can impair healing and predispose to fibrosis or infection.

Several conditions may present similarly to traumatic penile injuries and require differentiation for appropriate management. Squamous cell carcinoma may present with ulcerated masses resembling traumatic wounds or may be discovered incidentally when examining the penis following unrelated trauma. Habronemiasis produces granulomatous lesions that may appear similar to healing wounds or infected injuries. Balanoposthitis from infectious or irritant causes can produce swelling and discharge that might be mistaken for infection secondary to trauma. Papillomatosis or other viral infections can cause lesions that might be confused with minor traumatic wounds. Careful examination and appropriate diagnostic testing distinguish these conditions from traumatic injuries.

Potential complications of penile trauma extend beyond the immediate injury and influence long-term outcomes. Wound infection represents the most common complication, potentially progressing to abscess formation or cellulitis if not appropriately treated. Urethral stricture can develop following urethral injuries, causing ongoing difficulty with urination that may require repeated or permanent treatment. Fibrosis within the corpus cavernosum from injury or hematoma can result in erectile dysfunction affecting breeding capability. Abnormal scarring may cause cosmetic changes or rarely functional impairment from tissue distortion. Secondary paraphimosis from swelling, if not properly managed, can result in permanent tissue damage beyond that from the original injury.
