Pedal osteitis refers to inflammation and demineralization of the coffin bone (third phalanx or P3), the crescent-shaped bone enclosed within the horse's hoof capsule. This condition represents a reactive response of the distal phalanx to various insults including chronic bruising, concussive trauma, infection, or laminitis, resulting in bone resorption that appears as demineralization on radiographs. The term itself has been subject to debate within the veterinary community, as the radiographic changes observed may not always correlate directly with clinical lameness, making diagnosis and treatment decisions complex.
Pedal osteitis affects horses across breeds and disciplines but occurs most frequently in horses with thin soles, flat feet, or those working consistently on hard surfaces. Thoroughbreds and other breeds selected for speed often have thinner sole depth, predisposing them to concussive injury. Working horses, including police mounts and carriage horses exposed to paved surfaces, face elevated risk. Any horse subjected to repeated sole bruising, poor hoof care, or chronic foot conditions may develop pedal osteitis as a secondary consequence of ongoing inflammation.
The impact of pedal osteitis on equine health and performance ranges from subclinical radiographic changes without apparent lameness to significant chronic foot pain limiting athletic function. Horses with clinically significant pedal osteitis typically demonstrate sensitivity to hoof testers over the sole region, shortened stride length, and reluctance to work on hard surfaces. Performance may be particularly affected in disciplines requiring concussive activity such as jumping or working on hard arena footing. Bilateral involvement is common, sometimes making lameness appreciation difficult until nerve blocks localize pain to the feet.
Treatability of pedal osteitis depends on the underlying cause, severity of bone changes, and the horse's intended use. When identified early and addressed through appropriate management changes, many horses improve significantly and return to comfortable function. However, advanced cases with substantial bone loss may prove refractory to treatment, requiring ongoing management to maintain acceptable comfort levels. Early detection through recognition of subtle lameness signs and appropriate radiographic evaluation allows intervention before permanent structural changes limit treatment options and athletic potential.
