Patent urachus is a congenital condition in foals characterized by persistent patency of the urachus, a fetal structure that normally closes at birth, resulting in urine dribbling from the umbilicus. During fetal development, the urachus connects the bladder to the allantois, allowing fetal urine to drain into the allantoic fluid. At birth, this tubular structure should collapse and functionally close as the foal begins urinating normally through the urethra. When the urachus fails to close completely, urine continues to leak through the umbilical remnant, creating the condition known as patent or pervious urachus. This condition requires veterinary attention to prevent complications and ensure proper closure.
Patent urachus is among the more common umbilical abnormalities encountered in neonatal foals, occurring in an estimated 2-5% of newborns depending on the population studied. The condition may be present at birth (congenital patent urachus) or may develop within the first few days to weeks of life when an initially closed urachus reopens, often in association with illness or straining. Premature and dysmature foals face elevated risk due to incomplete development of umbilical structures and reduced tissue integrity. Foals with concurrent illness, particularly those causing straining from diarrhea or respiratory disease, may develop acquired patency.
The impact of patent urachus on foal health depends on whether the condition remains uncomplicated or progresses to infection. Simple patent urachus with urine drainage but no infection may be a relatively minor problem that responds well to conservative management. However, the continuous moisture at the umbilicus creates ideal conditions for bacterial colonization and ascending infection, which can spread to involve other umbilical structures and potentially the bloodstream. Omphalophlebitis, omphalitis, and septicemia represent serious complications that significantly increase morbidity and mortality.
Early recognition and appropriate management of patent urachus are essential for optimal outcomes. Prompt veterinary evaluation allows assessment for associated umbilical abnormalities and systemic illness, guides treatment selection, and enables monitoring for complications. Most foals with patent urachus respond well to treatment and recover fully, though those with significant concurrent illness or delayed treatment face greater risk of complications requiring more intensive management.
