Lipoma strangulation is a severe, life-threatening form of colic in horses caused when a pedunculated lipoma (a benign fatty tumor on a stalk) wraps around a segment of intestine, cutting off blood supply and creating a strangulating obstruction. Lipomas are benign growths composed of fatty tissue that commonly develop within the mesentery, the membranous tissue that supports and suspends the intestines within the abdominal cavity. These tumors are extremely common in horses over 15 years of age, with some studies suggesting that up to 50 percent of older horses have lipomas present at necropsy. While the lipomas themselves are harmless, those that develop on elongated stalks (pedunculated lipomas) can swing freely within the abdomen and become wrapped around intestinal segments.
Lipoma strangulation represents one of the most common causes of strangulating small intestinal lesions in older horses, accounting for approximately 20 to 30 percent of small intestinal surgeries in horses over 15 years of age. The condition shows clear demographic patterns, with dramatically increased incidence in senior horses, geldings, and certain breeds including ponies, Arabians, and American Saddlebreds. The tumor's stalk can wind around the small intestine, small colon, or occasionally the large colon, creating a tourniquet effect that completely cuts off blood supply to the affected segment. Without emergency surgical intervention, the entrapped intestine rapidly becomes devitalized, progressing to necrosis and potential rupture within hours.
The impact of lipoma strangulation on equine health is severe and acute, representing a true surgical emergency. Once strangulation occurs, horses develop rapidly progressive colic signs that do not respond to medical management. The affected intestinal segment suffers immediate vascular compromise, with venous return blocked first, leading to congestion and edema, followed by arterial compromise and complete ischemia. Tissue death begins within one to three hours of complete vascular occlusion, with irreversible damage occurring rapidly. The timeline from symptom onset to intestinal devitalization and potential rupture may be as short as six to eight hours, making rapid recognition and surgical intervention critical.
Early recognition of strangulating colic and prompt surgical intervention offer the best chance for survival. Horse owners should understand that any colic in a senior horse warrants serious attention and prompt veterinary evaluation, as lipoma strangulation can progress from initial discomfort to life-threatening emergency within hours. While surgical survival rates have improved significantly with advances in equine surgery and critical care, the best outcomes occur when surgery is performed before significant intestinal damage has occurred.
