Large strongyles, historically known as bloodworms or large redworms, represent a group of parasitic nematodes that were once the most significant internal parasites of horses due to their potential to cause life-threatening vascular damage and colic. Three species comprise this group: Strongylus vulgaris, the most pathogenic species causing arterial damage; Strongylus edentatus, which migrates through the liver and peritoneum; and Strongylus equinus, which also undertakes hepatic migration. Unlike their smaller cyathostomin relatives, large strongyles undergo extensive tissue migration during their larval stages, causing substantial damage to blood vessels, liver, and abdominal structures.
The prevalence of large strongyle infections has declined dramatically over recent decades due to the widespread use of effective anthelmintic medications, particularly ivermectin introduced in the 1980s. Where once nearly universal in horse populations, large strongyles now occur less frequently, though they remain present in horses receiving inadequate parasite control and in feral or semi-feral populations. Young horses demonstrate highest susceptibility, though horses of any age lacking appropriate deworming protection remain at risk. Geographic distribution spans all regions where horses graze, with no climate-related limitations to this cosmopolitan parasite group.
The impact of large strongyle infection on equine health can range from subclinical in light infections to catastrophic when larval migration damages critical arterial structures. Strongylus vulgaris larvae migrate through the mesenteric arterial system, particularly the cranial mesenteric artery, causing verminous arteritis with thrombosis and potential embolism to the intestinal blood supply. This vascular damage underlies the thromboembolic colic syndrome historically responsible for significant equine mortality. Liver damage from S. edentatus and S. equinus migration causes systemic effects including weight loss and poor condition. Heavy infections produce anemia, debilitation, and death if untreated.
Treatment with modern macrocyclic lactone anthelmintics effectively eliminates large strongyles at all life stages, including the migrating larvae most responsible for pathology. Prevention through appropriate deworming protocols has dramatically reduced disease incidence, though vigilance remains necessary as parasite populations persist. The success of large strongyle control represents one of veterinary medicine's significant parasite management achievements, though declining awareness as the condition becomes less common creates risk of resurgence should deworming practices deteriorate.
