Hydronephrosis is a condition characterized by dilation of the renal pelvis and calyces due to obstruction of urine flow, resulting in progressive swelling of the affected kidney. In horses, this condition develops when urine cannot drain properly from the kidney into the ureter and ultimately the bladder, causing backup pressure that gradually distends the kidney's collecting system. As the condition progresses, the accumulated urine pressure damages the delicate kidney tissue, potentially leading to irreversible loss of renal function in the affected kidney. Hydronephrosis may be unilateral, affecting only one kidney, or bilateral, involving both kidneys with more severe consequences for overall renal function.
Hydronephrosis in horses is relatively uncommon but can occur secondary to various obstructive conditions affecting the urinary tract. The condition may develop at any age, from congenital malformations causing obstruction in foals to acquired causes in adult horses such as urolithiasis, neoplasia, or stricture formation. Unilateral hydronephrosis may go undetected for extended periods because the contralateral kidney compensates for the affected kidney's diminished function. Bilateral obstruction, while less common, presents more acutely with signs of renal failure as both kidneys lose their ability to produce and excrete urine normally.
The impact of hydronephrosis on equine health depends on whether one or both kidneys are affected, the degree of obstruction, and the duration before diagnosis and treatment. Unilateral cases may cause no clinical signs if the opposite kidney functions normally, and may only be discovered incidentally during examination for other conditions. However, the affected kidney gradually loses function as prolonged obstruction destroys nephrons, and if bilateral disease develops or the remaining kidney fails, life-threatening renal insufficiency results. Performance horses with hydronephrosis may show subtle decreases in stamina or nonspecific signs of discomfort.
Early detection and treatment of hydronephrosis are essential for preserving kidney function. Relief of obstruction before irreversible damage occurs can allow significant recovery of renal function, while delayed treatment results in permanent kidney damage despite correction of the underlying obstruction. Veterinary evaluation of any horse with signs of urinary tract disease, abdominal pain, or nonspecific illness is important for identifying hydronephrosis before advanced changes develop.
