This disease occurs when horses inhale fungal spores (microconidia) that become airborne from contaminated soil, with infection ranging from subclinical exposure to severe, disseminated disease affecting multiple organ systems. While considered relatively uncommon in horses compared to dogs and cats in endemic areas, histoplasmosis represents an important differential diagnosis for horses presenting with chronic respiratory disease, weight loss, or unexplained systemic illness in regions where the organism is prevalent.
Histoplasmosis occurs in horses primarily in endemic areas where environmental conditions support Histoplasma growth, including the Ohio and Mississippi River valleys of the United States, parts of Central and South America, Africa, Southeast Asia, and Australia. The fungus thrives in soil enriched with nitrogen from accumulated bird or bat droppings, with particularly high concentrations found in areas around chicken houses, bird roosts, caves inhabited by bats, and old buildings where birds or bats have nested. True incidence in horses is unknown, as many infections likely remain subclinical and undiagnosed, with clinical disease representing a small fraction of horses exposed to the organism.
The impact of histoplasmosis on equine health varies considerably depending on spore exposure level, host immune status, and whether infection remains localized or disseminates systemically. Many exposed horses develop self-limiting infections that resolve without clinical illness. Horses with clinical disease may experience chronic respiratory compromise, progressive weight loss, lymph node enlargement, and multi-organ involvement with potentially serious consequences. Disseminated histoplasmosis affecting multiple organs carries guarded prognosis and may prove fatal despite treatment. The condition can significantly impair performance and quality of life when clinical disease develops.
With appropriate diagnosis and antifungal treatment, many horses with histoplasmosis can achieve clinical recovery, though treatment duration often extends for months and some cases prove refractory. Early recognition and intervention generally improves prognosis compared to advanced, disseminated disease.
