Fungal keratitis, also known as keratomycosis, is a serious fungal infection of the cornea that poses a significant threat to equine vision. This condition represents one of the most challenging ocular infections to treat in horses, often requiring prolonged, intensive medical therapy and frequently resulting in some degree of permanent corneal damage. The fungi responsible for keratomycosis are typically opportunistic organisms found commonly in the environment, particularly in soil, plant material, and organic debris, making horses' exposure to these pathogens essentially ubiquitous.
The prevalence of fungal keratitis varies geographically, with significantly higher incidence in warm, humid climates where environmental conditions favor fungal growth. Regions with hot summers and mild winters, particularly in the southeastern United States and similar climates worldwide, see disproportionately more cases. Horses of all breeds, ages, and uses may be affected, though those with corneal injuries that provide a portal of entry for fungal organisms are at particular risk. Horses kept on pasture or in environments with abundant plant material and organic debris face greater exposure to potential pathogens.
The impact of fungal keratitis on equine health is substantial, as this infection threatens not only the vision in the affected eye but potentially the eye itself. Fungal infections of the cornea tend to be more invasive and destructive than bacterial corneal infections, with a propensity to penetrate deep into the corneal stroma and potentially the anterior chamber. The inflammatory response to fungal infection is often severe, leading to significant pain and frequently accompanying uveitis. Without appropriate treatment, fungal keratitis can progress to corneal perforation and loss of the eye. Even with treatment, scarring and visual impairment are common sequelae.
Fungal keratitis is treatable but demands early recognition, accurate diagnosis, and aggressive, prolonged antifungal therapy. Treatment typically extends for weeks to months, requiring significant commitment from owners for intensive medication schedules. Despite appropriate treatment, the prognosis for complete visual recovery is guarded, with many horses retaining some degree of corneal opacity. Early detection before deep corneal invasion occurs offers the best chance for favorable outcomes. Any horse with a corneal ulcer that fails to respond to standard antibiotic therapy should be evaluated for fungal involvement.
