Duodenitis-Jejunitis Syndrome, commonly abbreviated as DJS and also known as anterior enteritis or proximal enteritis, is an acute inflammatory condition affecting the small intestine in horses. This syndrome specifically targets the duodenum and jejunum, the proximal portions of the small intestine immediately following the stomach. The inflammation causes massive fluid secretion into the intestinal lumen, cessation of normal forward intestinal motility, and accumulation of large volumes of fluid that reflux into the stomach. This gastric distension produces severe abdominal pain and places the horse at risk for potentially fatal gastric rupture if not properly managed.
Horses of all ages, breeds, and disciplines can develop DJS, and cases occur throughout the year, though some regions report increased incidence during warmer months. The condition appears sporadically without clear pattern of horse-to-horse transmission, though outbreaks affecting multiple horses on the same farm have been reported. Adult horses are more commonly affected than foals, and there is no clear sex predilection. The syndrome has been recognized for decades, yet the specific triggering factors remain incompletely understood in most individual cases.
The impact of DJS on affected horses is severe and potentially life-threatening. The combination of intestinal dysfunction, massive fluid sequestration, endotoxin absorption, and intense abdominal pain creates a critical medical emergency. Horses can deteriorate rapidly, and without aggressive treatment including nasogastric decompression, intravenous fluid therapy, and supportive care, the condition is frequently fatal. Even with optimal treatment, complications including laminitis, adhesion formation, and prolonged hospitalization are common. Mortality rates vary in reported studies but can approach or exceed twenty percent.
Successful management of DJS depends on early recognition, prompt initiation of appropriate therapy, and careful monitoring throughout the disease course. The syndrome can initially resemble surgical colic conditions, requiring careful differentiation to avoid unnecessary surgery. Understanding the characteristic clinical features of DJS helps veterinarians make appropriate diagnostic and treatment decisions. With aggressive supportive care, most horses with DJS can survive, though the recovery period is often prolonged and complications may affect long-term prognosis.
