The cauda equina, Latin for horse's tail, describes the bundle of nerve roots extending beyond the conus medullaris in the lumbosacral region, which innervate structures including the tail, perineum, bladder, rectum, and portions of the hindlimbs. When these nerve roots are damaged through various disease processes, affected horses develop characteristic signs including tail paralysis, urinary and fecal incontinence, and perineal sensory loss.
Cauda equina syndrome affects horses of all breeds and can develop at any age, though adult horses are most commonly diagnosed. The condition encompasses several distinct disease processes including polyneuritis equi, an immune-mediated inflammatory disease; neoplasia affecting the cauda equina region; trauma to the sacrocaudal spine; and infectious processes including equine herpesvirus myeloencephalopathy and bacterial infections. The specific underlying cause significantly influences treatment options and prognosis.
The impact of cauda equina syndrome on equine health extends beyond the obvious neurological deficits to encompass significant quality of life considerations. Urinary and fecal incontinence create management challenges and predispose to secondary complications including urine scalding, skin infections, and ascending urinary tract infections. Reduced hindquarter sensation increases injury risk. Sexual function may be compromised in breeding animals.
