Anterior enteritis, also known as proximal enteritis or duodenitis-proximal jejunitis, is an inflammatory condition affecting the duodenum and proximal portion of the jejunum in horses. This disease causes severe inflammation of the proximal small intestinal wall, leading to massive fluid and electrolyte secretion into the intestinal lumen. The result is functional obstruction of the gastrointestinal tract, with accumulation of large volumes of fluid in the stomach and small intestine. Horses with anterior enteritis typically present with colic, depression, and production of enormous quantities of nasogastric reflux when tubed, distinguishing this condition from many other colic causes.
The prevalence of anterior enteritis varies geographically, with significantly higher incidence reported in the southeastern United States compared to other regions. This geographic distribution suggests environmental or management factors may contribute to disease development, though the exact reasons for regional variation remain incompletely understood. Horses of any age, breed, or use may be affected, though adults are more commonly diagnosed than foals. The condition occurs sporadically, though occasional clustering of cases on farms suggests possible infectious or environmental triggers.
The impact of anterior enteritis on affected horses ranges from self-limiting illness requiring brief supportive care to life-threatening disease requiring prolonged intensive treatment. The massive fluid losses into the gastrointestinal tract lead to severe dehydration and electrolyte derangements if not aggressively addressed. Secondary complications including laminitis and thrombophlebitis develop in some cases, significantly affecting prognosis and recovery. Horses that survive without complications typically recover fully, while those developing secondary problems may face prolonged rehabilitation or permanent sequelae.
Anterior enteritis is treatable through medical management in most cases, distinguishing it from many surgical colic conditions. The mainstay of treatment involves fluid therapy to replace losses, nasogastric decompression to relieve gastric distension, and supportive care while the intestinal inflammation resolves. Distinguishing anterior enteritis from surgical conditions like small intestinal strangulation is critical, as the treatments differ completely and unnecessary surgery worsens outcomes for enteritis cases. Most horses with anterior enteritis survive with appropriate treatment, though the disease requires intensive care over several days and carries risk of serious complications.
