Hypocalcemia, commonly known as milk fever in cattle or lambing sickness in sheep, is a severe metabolic disorder characterized by dangerously low levels of calcium in the blood that occurs primarily around the time of parturition and early lactation. Despite its common name, milk fever does not involve an elevated body temperature; rather, it refers to the acute depletion of blood calcium that occurs when the sudden demands of milk production exceed the animal's ability to mobilize calcium from bones and absorb it from the diet. This condition represents one of the most significant metabolic emergencies in dairy production and can be rapidly fatal if not treated promptly with intravenous calcium supplementation.
Milk fever predominantly affects dairy cattle, particularly high-producing cows in their third or subsequent lactations, though it also occurs in sheep, goats, and occasionally other ruminant species. In dairy cattle, the condition typically develops within the first 72 hours after calving, with the highest incidence occurring within 24 hours of delivery. Sheep and goats are most commonly affected in late pregnancy, particularly when carrying multiple fetuses, though postpartum cases also occur. The condition is relatively common in intensive dairy production systems, with incidence rates in dairy cattle herds ranging from 5% to over 10% in some populations, while subclinical hypocalcemia (low blood calcium without obvious clinical signs) may affect 50% or more of older dairy cows at calving.
The economic and welfare impact of hypocalcemia in dairy production is substantial. Direct losses include animal deaths, which can occur within hours if treatment is not provided, as well as costs of veterinary treatment and the labor required for intensive monitoring and care of affected animals. Indirect economic impacts may be even greater, as hypocalcemia is associated with increased risk of numerous other periparturient disorders including ketosis, displaced abomasum, retained placenta, metritis, and mastitis. Subclinical hypocalcemia, even without obvious clinical signs, reduces rumen function, depresses feed intake, impairs immune function, and decreases milk production, creating cascading effects on overall herd productivity and animal welfare.
With prompt recognition and appropriate treatment, clinical milk fever has an excellent prognosis, with most affected animals responding dramatically to intravenous calcium therapy and returning to normal function within hours. However, prevention is far preferable to treatment, and modern dairy management places significant emphasis on nutritional strategies during the dry period and transition phase to support calcium metabolism and prevent hypocalcemia. Understanding the underlying physiology of calcium regulation and implementing evidence-based prevention protocols can dramatically reduce the incidence of this condition and its associated complications, making milk fever prevention a cornerstone of successful transition cow management programs.
