Hypocalcemia, commonly known as milk fever or parturient paresis, is an acute metabolic emergency occurring in dairy cattle and other ruminants when blood calcium levels drop to dangerously low levels around the time of calving. Despite the misleading common name, this condition involves neither fever nor mammary gland infection, but rather a critical shortage of calcium in the bloodstream that impairs normal neuromuscular function throughout the body. The sudden massive demand for calcium to produce colostrum and milk at the onset of lactation overwhelms the body's ability to mobilize calcium from bones and absorb it from the diet, resulting in a potentially fatal metabolic crisis.
Milk fever primarily affects dairy cattle, with the highest incidence occurring in high-producing cows entering their third or subsequent lactations. The condition affects an estimated 5 to 10 percent of dairy cows in affected herds, though subclinical hypocalcemia (low blood calcium without obvious clinical signs) may affect 50 percent or more of periparturient dairy cows. Sheep and goats also develop hypocalcemia, typically in late pregnancy when fetal calcium demands are highest, a condition often called pregnancy toxemia or twin lamb disease when combined with energy deficiency. The timing and presentation differ somewhat between species, but the underlying calcium deficiency and resulting neuromuscular dysfunction are similar.
The economic and welfare impact of hypocalcemia extends far beyond the immediate emergency. Direct costs include treatment expenses, veterinary fees, and mortality losses in severe cases. Indirect costs are substantial and include increased risk of numerous secondary conditions including displaced abomasum, retained placenta, metritis, mastitis, and ketosis. Cows affected by hypocalcemia have reduced milk production throughout lactation, impaired fertility, and increased risk of culling. The immunosuppressive effects of low calcium contribute to increased disease susceptibility during the critical early lactation period. Estimates suggest hypocalcemia costs the dairy industry hundreds of millions of dollars annually in the United States alone.
Recognition of early clinical signs and immediate treatment with intravenous calcium are essential for successful outcomes in milk fever cases. The classic presentation of a recently calved cow unable to rise is unmistakable, but earlier detection allows treatment before the animal becomes recumbent and at risk for secondary complications. Prevention strategies targeting calcium metabolism around calving have dramatically reduced milk fever incidence in herds implementing these approaches and represent the most cost-effective approach to this common and costly condition.
