Abomasal volvulus is a life-threatening surgical emergency in cattle characterized by rotation or twisting of the abomasum around its mesenteric axis, resulting in vascular compromise, tissue necrosis, and rapid systemic deterioration. This condition represents the most severe manifestation of abomasal positional disorders, typically developing as a progression from right displaced abomasum (RDA) when the gas-distended organ rotates beyond 180 degrees. The rotation obstructs blood flow to the abomasal wall and may also compromise the adjacent omasum and proximal duodenum, leading to tissue death within hours if not surgically corrected. Abomasal volvulus constitutes a true veterinary emergency requiring immediate recognition and intervention for any chance of survival.
Abomasal volvulus occurs predominantly in dairy cattle populations, sharing the same general risk profile as abomasal displacement but with additional factors that predispose to the rotational progression. The condition most commonly affects cattle in the first several weeks after calving, during the same high-risk transition period associated with simple displacement. While left displaced abomasum (LDA) is more common overall, right-sided displacements carry significantly higher risk of progression to volvulus, making RDA a more serious diagnosis despite its lower frequency. Estimated incidence of volvulus ranges from 0.2% to 0.5% of calvings in high-risk dairy populations, representing a smaller subset of the animals affected by any form of abomasal displacement.
The economic and welfare impact of abomasal volvulus is severe on both individual animal and herd levels. Mortality rates remain high despite aggressive surgical intervention, with survival rates ranging from 40% to 75% depending on case severity and treatment timing. Animals that die or require euthanasia represent complete losses of their genetic and productive value. Survivors face prolonged recovery periods and may never return to full production capacity. Direct costs include emergency veterinary services, intensive surgical intervention, extended post-operative care, and medications required for recovery. Indirect losses include milk production foregone during illness and recovery, extended days open affecting reproductive efficiency, and increased culling probability even among survivors.
Early recognition of volvulus symptoms and immediate veterinary intervention are absolutely critical for survival. The clinical presentation of abomasal volvulus differs importantly from simple displacement, with more severe signs of systemic illness including shock, dehydration, and cardiovascular compromise. Producers and farm personnel must understand these distinguishing features to ensure emergency response rather than routine scheduling of veterinary care. Every hour of delay significantly reduces survival probability as tissue damage progresses and systemic toxemia worsens. Farms with high rates of abomasal displacement should establish emergency protocols ensuring rapid veterinary contact when volvulus is suspected, potentially including 24-hour emergency services or on-call arrangements with surgical referral centers.
