Tricuspid valve dysplasia is a congenital cardiac malformation in which the tricuspid valve, the structure separating the right atrium from the right ventricle, develops abnormally during fetal growth. The resulting structural defects compromise the valve's ability to close properly during ventricular contraction, allowing blood to flow backward from the right ventricle into the right atrium. This backward flow, known as tricuspid regurgitation, places increasing strain on the right side of the heart and can ultimately lead to right-sided congestive heart failure.
The tricuspid valve is a complex structure composed of three leaflets, a fibrous annulus, chordae tendineae, and papillary muscles, all of which must develop and function in precise coordination to ensure unidirectional blood flow. In TVD, one or more components of this apparatus are malformed. The valve leaflets may be thickened, shortened, or fused to the ventricular wall. The chordae tendineae, the fibrous cords that tether the leaflets to the papillary muscles and prevent them from prolapsing into the atrium during contraction, may be shortened, thickened, absent, or abnormally attached. The annulus itself may be dilated, preventing the leaflets from coapting even if their individual structure is relatively preserved.
TVD is one of the more commonly diagnosed congenital cardiac defects in dogs, though its overall prevalence in the general canine population remains relatively low. The condition accounts for approximately seven to eight percent of all congenital heart defects identified in dogs. Males appear to be affected slightly more frequently than females in several studied breeds, though the sex predisposition is not absolute and varies among breeds.
The clinical significance of TVD ranges enormously depending on the severity of the valvular malformation. Dogs with mild dysplasia may live normal or near-normal lifespans with minimal clinical signs, while those with severe malformations can develop progressive right-sided heart failure in the first year of life. This spectrum of severity means that TVD encompasses everything from an incidental murmur discovered during routine examination to a life-limiting cardiac condition requiring intensive management.
