Underlying systemic conditions frequently contribute to the development of chronic ear disease and subsequent otitis media. Topical therapy may be directed into the middle ear following lavage, though the selection of topical agents must account for potential ototoxicity. Many commonly used otic medications contain aminoglycoside antibiotics, propylene glycol, or other ingredients that can damage cochlear and vestibular structures if they reach the inner ear. Aqueous-based preparations of fluoroquinolones or tris-EDTA solutions are generally considered safer for middle ear application. Topical corticosteroids may be used to reduce inflammation and edema within the middle ear.

Addressing underlying conditions is essential for preventing recurrence. Dogs with allergic skin disease should receive appropriate allergy management, which may include dietary trials, immunotherapy, or long-term anti-pruritic medications. Surgical Treatment

The choice of surgical procedure depends on the severity and chronicity of the disease, the condition of the ear canal, and the extent of bulla involvement.

Ventral bulla osteotomy is the most commonly performed surgical procedure for otitis media in dogs. This approach provides direct access to the tympanic bulla through a ventral incision in the neck, allowing the surgeon to open the bulla, remove infected material and thickened mucosa, obtain samples for culture, and establish drainage. The ventral approach provides good exposure of both compartments of the tympanic bulla and allows thorough curettage of the bulla lining. Potential complications include damage to the hypoglossal nerve, lingual artery, or sympathetic trunk during the surgical approach.

Total ear canal ablation with lateral bulla osteotomy, commonly referred to as TECA-LBO, is performed when both the ear canal and middle ear are severely diseased. This procedure removes the entire external ear canal along with the lateral wall of the tympanic bulla, eliminating the chronically infected ear canal while providing access to debride the middle ear. TECA-LBO is considered a salvage procedure and results in permanent deafness on the operated side. Despite this, most owners report significant improvement in their dog's quality of life following the procedure due to the elimination of chronic pain and infection.

Postoperative care following bulla surgery is intensive and includes systemic antimicrobial therapy based on culture results, pain management with multimodal analgesic protocols, anti-inflammatory medications, and careful wound management. Complications and Associated Conditions

Basset Hounds share similar predispositions due to their extremely long ear flaps and narrow canals.
