Otitis media, commonly abbreviated as OM, refers to inflammation and infection of the middle ear in dogs. The middle ear is a small air-filled chamber housed within the tympanic bulla, a bony structure located at the base of the skull. This chamber contains the auditory ossicles (the malleus, incus, and stapes), which are responsible for transmitting sound vibrations from the tympanic membrane (eardrum) to the inner ear. When infection or inflammation develops in this space, it can compromise hearing, affect balance, and damage adjacent neural structures.
Otitis media in dogs most commonly develops as an extension of chronic or severe otitis externa, where infection from the external ear canal penetrates through the tympanic membrane into the middle ear cavity. Studies have demonstrated that a significant percentage of dogs with chronic otitis externa also have concurrent otitis media, often undiagnosed. This makes OM an underrecognized condition that contributes to treatment failure in dogs with seemingly refractory ear disease.
The condition can also arise through the auditory tube (Eustachian tube), which connects the middle ear to the nasopharynx, though this route of infection is less common in dogs than in humans. Hematogenous spread of infection, where bacteria reach the middle ear through the bloodstream, is a rare but documented pathway. In some cases, trauma, foreign bodies, or neoplasia can compromise the integrity of the tympanic membrane and allow organisms to access the middle ear.
Otitis media is clinically significant not only because of the direct effects of middle ear infection but also because of its potential to affect structures that pass through or adjacent to the tympanic bulla. The sympathetic nerve fibers to the eye, the facial nerve, and the vestibulocochlear nerve all have anatomical relationships with the middle ear. Damage to these structures can produce Horner's syndrome, facial nerve paralysis, and vestibular dysfunction, respectively.
Early recognition and aggressive treatment of otitis media are essential to prevent progression to otitis interna (inner ear infection), permanent hearing loss, and chronic neurological complications. The condition requires a systematic diagnostic approach and often demands prolonged therapy to achieve resolution.
