Osteochondritis Dissecans, commonly abbreviated as OCD, is a developmental orthopedic condition in which the normal process of cartilage maturation into bone is disrupted within a growing dog's joints. During healthy skeletal development, a layer of cartilage at the joint surface gradually transforms into bone through a process called endochondral ossification. In dogs with OCD, this process fails in focal areas, resulting in abnormally thickened cartilage that does not receive adequate nutrition from the underlying bone.
As the thickened cartilage loses its nutritional support, it becomes weakened and susceptible to damage. Over time, cracks develop in the abnormal cartilage, and a flap or fragment may partially or completely separate from the underlying subchondral bone. This loose or partially attached cartilage flap causes significant inflammation within the joint, leading to pain, effusion, and progressive lameness. The condition most commonly appears in rapidly growing large and giant breed dogs between four and eight months of age.
OCD is distinct from simple osteochondrosis, which refers to the broader category of abnormal cartilage development. The term osteochondritis dissecans specifically denotes the stage at which a cartilage flap has formed and is causing clinical signs. Not all cases of osteochondrosis progress to OCD, and some dogs with mild cartilage irregularities may never show symptoms. However, once a dissecting flap develops, spontaneous resolution is uncommon, and veterinary intervention is typically required.
The condition can affect multiple joints, though the shoulder is the most frequently involved site, accounting for a large proportion of diagnosed cases. The caudal humeral head is the classic location within the shoulder joint. The medial humeral condyle of the elbow, the lateral or medial femoral condyle of the stifle, and the medial trochlear ridge of the talus in the hock are other well-documented locations. Bilateral involvement, where both corresponding joints are affected, occurs in a significant percentage of cases.
