Neck-scratcher's disease is the colloquial name for the neurological condition caused by syringomyelia secondary to Chiari-like malformation in dogs. This condition involves a structural abnormality of the skull in which the caudal fossa, the compartment at the back of the skull that houses the cerebellum and brainstem, is too small relative to its neural contents. The resulting overcrowding causes herniation of the cerebellar tissue through the foramen magnum, the opening at the base of the skull where the brainstem transitions into the spinal cord.
This herniation obstructs the normal flow of cerebrospinal fluid between the brain and the spinal canal. The disrupted fluid dynamics lead to the formation of fluid-filled cavities, called syrinxes, within the substance of the spinal cord itself. These syrinxes damage the neural tissue of the spinal cord, producing a range of neurological signs including the characteristic phantom scratching behavior that gives the condition its common name.
The condition was first described in detail in Cavalier King Charles Spaniels, in which it is extraordinarily prevalent, but it has since been recognized in a number of other toy and small breeds. The high prevalence in Cavaliers is directly related to selective breeding for certain skull conformations, specifically the rounded, dome-shaped skull and shortened face that are hallmarks of the breed standard. The genetic basis of the condition means that it runs strongly through breeding lines.
Phantom scratching, the hallmark sign that led to the common name, involves the dog making scratching motions directed at the neck, ear, or shoulder area without the paw actually making contact with the skin. This behavior results from neuropathic sensations caused by syrinx damage to the dorsal horn of the cervical spinal cord. The dog perceives abnormal sensations such as tingling, burning, or pain in the affected dermatomes and attempts to scratch in response to these phantom stimuli.
Neck-scratcher's disease represents one of the most significant hereditary health concerns in certain breeds, and its prevalence has generated considerable discussion about breeding ethics, health screening, and the responsibilities of breed clubs and kennel organizations in addressing conformational health problems.
