Mitral valve dysplasia is a congenital cardiac defect in which the mitral valve and its associated structures develop abnormally during fetal life, resulting in a malformed valve apparatus that is present at birth. Unlike the far more common myxomatous mitral valve degeneration, which is an acquired condition that develops progressively in older dogs, mitral valve dysplasia is a developmental abnormality that exists from the moment the puppy is born. The malformation can affect any component of the mitral valve apparatus, including the valve leaflets, the annulus, the chordae tendineae, and the papillary muscles.
The normal mitral valve is a complex structure consisting of two leaflets, anterior and posterior, that are anchored to papillary muscles within the left ventricle by thin fibrous cords called chordae tendineae. During ventricular diastole, the valve opens to allow blood to flow from the left atrium into the left ventricle, and during systole, the leaflets close tightly to prevent backward flow. In mitral valve dysplasia, developmental anomalies disrupt this normal architecture, preventing the valve from functioning properly.
The specific morphological abnormalities seen in mitral valve dysplasia can vary considerably from one affected dog to another. Leaflet malformations may include thickened, shortened, or rolled valve leaflets that cannot achieve proper coaptation. Chordal abnormalities may involve shortened, elongated, thickened, or absent chordae that fail to properly tether the leaflets. The papillary muscles may be malpositioned, hypertrophied, or abnormally fused. In some cases, direct insertion of papillary muscle tissue into the valve leaflet occurs without intervening chordae, a configuration that severely restricts normal valve motion.
Mitral valve dysplasia is classified as a relatively uncommon congenital cardiac defect in dogs compared to patent ductus arteriosus, pulmonic stenosis, and subaortic stenosis. However, it represents a significant cause of mitral regurgitation in young dogs, particularly in large breed dogs where acquired degenerative valve disease is less common. The condition may occur in isolation or in combination with other congenital cardiac malformations, and the severity of hemodynamic compromise varies widely depending on the specific structural abnormalities present.
