Metritis is a serious bacterial infection of the uterus that occurs in female dogs, most commonly developing within the first one to two weeks following whelping or giving birth. This acute inflammatory condition affects the endometrial lining of the uterus and can rapidly progress to a life-threatening systemic infection if not treated promptly. Unlike pyometra, which typically develops weeks to months after a heat cycle, metritis is specifically associated with the postpartum period and represents a veterinary emergency requiring immediate medical attention. The condition occurs when bacteria ascend through the open cervix during or after delivery and colonize the compromised uterine tissue.
The development of metritis is closely linked to complications during the birthing process. Dystocia, or difficult labor, creates an environment where the uterus is stressed and potentially traumatized, making it susceptible to bacterial invasion. Retained placentas or fetal membranes provide an ideal medium for bacterial growth, while prolonged labor weakens the natural defense mechanisms of the reproductive tract. The bacteria most commonly involved include Escherichia coli, Staphylococcus species, Streptococcus species, and various anaerobic organisms that normally inhabit the vaginal tract but become pathogenic when introduced to the compromised uterine environment.
The impact of metritis on a dog's health can be devastating and extends beyond the reproductive system to affect the entire body. As bacteria multiply within the uterus, they release toxins that enter the bloodstream, leading to septicemia and potentially septic shock. This systemic infection can cause organ dysfunction, particularly affecting the kidneys and liver, and can rapidly become fatal without aggressive treatment. Additionally, affected dogs are unable to properly care for their nursing puppies, putting the entire litter at risk of illness, hypothermia, and starvation if the mother becomes too ill to nurse.
The prognosis for dogs with metritis depends heavily on how quickly the condition is diagnosed and treated. When caught early and treated aggressively with appropriate antibiotics and supportive care, many dogs recover fully and can even go on to have future litters if desired. However, delayed treatment significantly worsens the prognosis, and some dogs may require emergency spaying to save their lives. Owners of breeding dogs should be aware of the signs of metritis and seek veterinary care immediately if they notice any concerning symptoms in their postpartum female, as early intervention is critical for the best possible outcome.
