Melanosis oculi is a progressive condition characterized by the excessive accumulation and proliferation of melanocytes (pigment-producing cells) within the structures of the eye. Unlike melanoma, which is a malignant neoplasm of melanocytes, melanosis oculi is classified as a non-neoplastic proliferative disorder, though the distinction can be challenging both clinically and histologically. The condition involves a diffuse increase in melanocytic cells throughout the uveal tract, which includes the iris, ciliary body, and choroid, and may also extend to the sclera, episclera, and extraocular tissues.
The clinical significance of melanosis oculi lies primarily in its potential to cause secondary glaucoma, a serious and vision-threatening complication. As melanocytes proliferate within the eye, pigment-laden cells and released melanin granules can infiltrate and obstruct the iridocorneal drainage angle, the structure responsible for outflow of aqueous humor from the eye. This obstruction leads to elevated intraocular pressure (IOP), which, if sustained, damages the optic nerve and retina, resulting in irreversible vision loss. The term pigmentary glaucoma is used to describe glaucoma that develops as a consequence of this pigment-mediated drainage angle obstruction.
Melanosis oculi has been most extensively studied and characterized in Cairn Terriers, in which the condition follows an autosomal dominant inheritance pattern. In this breed, the condition is bilateral and progressive, with clinical signs typically becoming apparent in middle age and advancing gradually over months to years. However, melanosis oculi has also been documented in several other breeds and in mixed-breed dogs, though the genetic basis in these populations is less well defined.
The condition exists on a spectrum, and not all affected dogs will develop glaucoma or require aggressive intervention. Some dogs maintain stable ocular pigmentation for years without significant elevation of intraocular pressure, while others progress rapidly to uncontrollable glaucoma requiring enucleation. This variability underscores the importance of regular ophthalmologic monitoring for all dogs diagnosed with melanosis oculi, regardless of the severity of pigmentation at the time of initial diagnosis.
