Legg-Calve-Perthes syndrome, also known as avascular necrosis of the femoral head, is an orthopedic condition in which the blood supply to the head of the femur, the ball portion of the hip's ball-and-socket joint, is disrupted. Without adequate blood flow, the bone tissue of the femoral head undergoes ischemic necrosis, meaning it dies and begins to deteriorate. As the bone weakens and collapses, the smooth spherical shape of the femoral head is lost, resulting in incongruity within the hip joint, progressive arthritis, pain, and lameness.
The condition predominantly affects young dogs of small and toy breeds, with the onset of clinical signs typically occurring between four and twelve months of age. The disease process actually begins weeks to months before clinical signs become apparent, as the bone undergoes a silent phase of necrosis before structural collapse creates the mechanical and inflammatory changes that produce pain and lameness. By the time a puppy begins limping, significant damage to the femoral head has usually already occurred.
Legg-Calve-Perthes syndrome is named after the three physicians who independently described the analogous condition in human children in the early 1900s: Arthur Legg, Jacques Calve, and Georg Perthes. The canine version of the disease closely parallels the human condition in its pathogenesis, clinical presentation, and treatment principles, though the surgical management differs somewhat between the two species due to anatomical and biomechanical differences.
Although the condition most commonly affects a single hip, bilateral involvement, meaning both hips are affected, has been documented in approximately 10 to 15 percent of cases. When bilateral disease is present, the severity may differ between the two sides, and the onset on the second side may be delayed relative to the first. Bilateral involvement can complicate treatment planning and rehabilitation, as the dog cannot shift weight entirely to an unaffected limb during recovery.
