Laryngeal paralysis is a progressive neurological condition affecting the larynx, or voice box, where the cartilages that normally open during breathing fail to function properly due to nerve dysfunction. The larynx serves as a gateway between the throat and the trachea, with paired cartilages called the arytenoids that should open wide during inhalation to allow adequate airflow. When the nerves controlling these cartilages fail, the arytenoids hang limply in a neutral or closed position, significantly restricting the airway and causing the characteristic noisy breathing, exercise intolerance, and voice changes associated with this condition. Laryngeal paralysis primarily affects older large breed dogs and can range from a mild inconvenience to a life-threatening emergency.
The condition is now recognized as part of a larger syndrome called geriatric onset laryngeal paralysis and polyneuropathy, or GOLPP, reflecting the understanding that the laryngeal nerve dysfunction is typically one manifestation of a more widespread degenerative nerve disease. The nerves controlling the larynx travel a long path from the brain down the neck and back up to the larynx, making them vulnerable to degeneration. As these nerves progressively fail, the muscles that open the arytenoid cartilages weaken and eventually cease to function. In many dogs, similar nerve degeneration affects other parts of the body, particularly the hind limbs, though laryngeal dysfunction is often the first and most obvious symptom.
The impact of laryngeal paralysis on affected dogs can be substantial, affecting their ability to exercise, tolerate heat, and engage in normal activities. Dogs with laryngeal paralysis must work harder to breathe, particularly during exertion, and the turbulent airflow through the narrowed larynx produces the characteristic loud, harsh breathing sounds that often first alert owners to a problem. The increased effort of breathing generates heat, and the compromised airway reduces heat dissipation through panting, making affected dogs dangerously susceptible to overheating. Voice changes, including a hoarse bark or complete loss of voice, commonly occur as the laryngeal muscles responsible for vocalization are affected.
Fortunately, effective surgical treatment exists for laryngeal paralysis, and many dogs can return to relatively normal function with appropriate intervention. The most common surgical procedure permanently secures one arytenoid cartilage in an open position, providing a fixed airway opening that allows adequate breathing without the need for active muscle function. Medical management and lifestyle modifications can help dogs with milder symptoms or those not suitable for surgery. Understanding the progressive nature of the underlying nerve disease helps owners and veterinarians develop appropriate management plans and realistic expectations for long-term outcomes.
