Haw's Syndrome is a condition characterized by the bilateral protrusion or elevation of the third eyelid, also known as the nictitating membrane or haw, in both eyes simultaneously. The third eyelid is a translucent to opaque tissue located at the inner corner of each eye that serves as an additional protective layer for the cornea. In its normal resting position, only a small portion of the third eyelid is visible. In Haw's Syndrome, both third eyelids rise significantly, partially covering the eyes and giving the dog an unusual, often alarming appearance.
The condition is named after the colloquial term for the third eyelid itself. While Haw's Syndrome is more commonly recognized and documented in cats, it does occur in dogs and is an important condition for veterinarians and pet owners to understand. The bilateral nature of the condition distinguishes it from unilateral third eyelid protrusion, which typically suggests a different set of underlying causes including localized orbital disease or unilateral Horner's syndrome.
Haw's Syndrome in dogs is generally considered a benign and self-limiting condition, though it often reflects an underlying process that warrants investigation. The protrusion itself does not typically cause pain or significant visual impairment, though severely elevated third eyelids can partially obstruct the dog's visual field. The mechanism behind the third eyelid elevation in Haw's Syndrome involves the autonomic nervous system, specifically an imbalance between the sympathetic and parasympathetic innervation of the smooth muscle that controls the position of the nictitating membrane. When sympathetic tone is reduced or parasympathetic activity predominates, the smooth muscle that normally holds the third eyelid in its retracted position relaxes, allowing the membrane to passively rise across the eye surface.
