What Is Eye Proptosis?

Eye proptosis, formally known as ocular proptosis or traumatic globe luxation, is a condition in which the eyeball is displaced forward out of the bony orbit and becomes trapped in front of the eyelids. The eyelids then close behind the globe, preventing the eye from returning to its normal position. This is a true veterinary emergency that requires immediate professional intervention to give the eye the best chance of survival.

The condition should be distinguished from exophthalmos, which refers to a more gradual forward bulging of the eye caused by a mass or swelling behind the globe within the orbit. In proptosis, the displacement is typically sudden and results from trauma or, in predisposed breeds, from relatively minor mechanical forces applied to the head or neck. The eyeball itself moves forward past the eyelid margins, and the lids then act as a constricting band behind the equator of the globe.

When the eye proptoses, the blood supply can be compromised by the eyelid constriction and by damage to the orbital vessels. The extraocular muscles that normally control eye movement may stretch, tear, or avulse from the globe. The optic nerve, which carries visual information from the retina to the brain, is placed under traction and may sustain irreversible damage. The cornea, now fully exposed and unprotected by the eyelids, rapidly dries and can develop ulceration.

By contrast, when proptosis occurs in a deep-orbited breed such as a German Shepherd, Labrador Retriever, or sighthound, the traumatic force involved is necessarily substantial, and the prognosis is correspondingly worse.

Orbital disease processes including abscesses, neoplasia, and cellulitis can weaken the orbital tissues and predispose to proptosis, though these are uncommon causes compared to direct trauma. Pre-existing ocular conditions that cause globe enlargement, such as chronic glaucoma with buphthalmos, may also lower the threshold for displacement.

Owners should not attempt to push the eye back into the socket. While the instinct to replace the globe is understandable, doing so without proper sedation, lubrication, and technique risks causing additional damage to the eye, extraocular muscles, and optic nerve. The eyelid margins must be manipulated properly to release the globe, and this requires the controlled environment of a veterinary facility.

The dog should be kept as calm and still as possible during transport. An Elizabethan collar, if available, can prevent the dog from pawing at the eye. If the dog is in significant pain, it may require careful muzzling to prevent biting during handling, though care must be taken not to apply pressure to the affected side of the face. The presence of hyphema, or blood within the anterior chamber, is noted as an indicator of intraocular damage. The condition of the cornea is evaluated for ulceration, perforation, or desiccation.

Skull radiographs or computed tomography may be performed to evaluate the bony orbit for fractures, particularly in cases involving significant blunt force trauma. These imaging modalities can also reveal retrobulbar hemorrhage or foreign material within the orbit. Globe Replacement Surgery

When the clinical assessment suggests a reasonable chance of globe survival, replacement of the eye into the orbit is performed as an emergency procedure. The dog is placed under general anesthesia, and the surgical field is prepared with gentle lavage to remove any debris. The cornea and conjunctiva are copiously irrigated with sterile saline, and the globe is carefully examined one final time before replacement.

The replacement procedure involves gently manipulating the eyelid margins over the equator of the globe to release the entrapment. A small opening is usually left at the medial canthus to allow application of topical medications. Systemic corticosteroids may be used cautiously to reduce inflammation and orbital edema, though their use must be balanced against the risk of impaired healing. The dog is fitted with an Elizabethan collar and discharged with detailed instructions for home care and medication administration.

Enucleation and When It Is Necessary

Enucleation, the surgical removal of the entire eyeball, is recommended when the prognosis for globe survival and useful vision is poor, or when the extent of damage makes preserving the eye inadvisable. Specific indications for enucleation include rupture of the globe, avulsion of the optic nerve, rupture of three or more extraocular muscles, severe and irreparable intraocular damage including total hyphema and lens dislocation, and corneal perforation with loss of intraocular contents.

The decision to enucleate can be emotionally difficult for owners, and veterinarians should communicate clearly and compassionately about the rationale. Owners should be prepared for this possibility and counseled that the goal of initial replacement is to give the eye the best possible chance, with the understanding that long-term outcomes cannot be guaranteed.

Corneal desiccation, progressive edema of the orbital tissues, and ischemic damage to the retina and optic nerve all worsen with time.

Even when vision is not preserved, many owners find the cosmetic outcome of a replaced globe preferable to enucleation, provided the eye remains comfortable. A retained but blind eye that is non-painful and cosmetically acceptable can be a satisfactory outcome for many families. However, if chronic complications develop that cause pain or require ongoing intensive management, delayed enucleation remains an option and should not be viewed as a failure of the initial treatment.

Prevention Strategies

Preventing eye proptosis requires awareness of breed-specific risk factors and proactive steps to minimize the likelihood of traumatic events. Owners of brachycephalic breeds should understand that their dogs' prominent eye anatomy makes them inherently vulnerable to globe displacement, and that even everyday activities carry some degree of risk. This awareness should inform decisions about handling, play, socialization, and equipment choices.

The use of harnesses rather than collars is strongly recommended for all brachycephalic breeds. Collars, particularly when attached to a leash and subjected to sudden pulling or jerking, can generate sufficient pressure on the neck to elevate orbital venous pressure and precipitate proptosis. A well-fitted harness distributes force across the chest and shoulders, eliminating this risk entirely. Body harnesses are also preferable for breeds prone to tracheal collapse, providing dual benefit.

Supervised interactions with other dogs, particularly larger or rough-playing dogs, are an important preventive measure. Scruffing, which involves grasping the loose skin at the back of the neck, should be avoided entirely in brachycephalic dogs as the traction forces can displace the globe. Veterinary professionals should be trained in gentle restraint techniques appropriate for these breeds. Owners should also be vigilant about household hazards, keeping the dog away from situations where it could sustain a blow to the head, and should consider crating during car travel to prevent injury in the event of a sudden stop or collision.
