Eosinophilic panosteitis is an inflammatory condition affecting the long bones of young, growing dogs. Commonly referred to as "pano" or "growing pains," this disease involves inflammation of the medullary cavity and endosteal lining of the diaphysis, or shaft, of one or more long bones. The condition derives its name from the characteristic presence of eosinophils observed in the bone marrow during certain stages of the disease process, although the precise role of these inflammatory cells in the pathogenesis remains a subject of ongoing investigation.
The disease typically affects dogs between five and eighteen months of age, with the peak incidence occurring between six and twelve months during the period of most rapid skeletal growth. While any breed can develop panosteitis, it is overwhelmingly a disease of large and giant breed dogs. German Shepherd Dogs are the most frequently affected breed, and males are affected approximately four times more often than females, although the reason for this sex predilection is not fully understood.
Panosteitis is fundamentally a self-limiting condition, meaning that it will eventually resolve on its own without treatment. However, the episodes of lameness and pain that characterize the disease can be significant and may recur over a period of weeks to months. Each episode typically lasts two to three weeks, and the lameness characteristically shifts from one leg to another between episodes, earning the condition its colloquial name of "wandering lameness" or "shifting leg lameness."
Despite its self-limiting nature, panosteitis should not be dismissed as trivial. The pain associated with active episodes can be substantial, and the condition must be differentiated from other causes of lameness in young dogs, some of which carry far more serious implications. Conditions such as hypertrophic osteodystrophy, osteochondritis dissecans, and bone tumors can present with similar clinical signs and require fundamentally different management approaches.
