Enostosis, more commonly known as panosteitis, is a self-limiting inflammatory condition of the long bones that primarily affects young, rapidly growing large and giant breed dogs. The condition is characterized by endosteal and periosteal bone proliferation within the medullary cavity of the long bones, causing pain that manifests as lameness. While the condition can be alarming to owners witnessing their otherwise healthy young dog suddenly become lame, enostosis is generally a benign condition that resolves on its own as the dog matures.
The condition was first described in the veterinary literature in the mid-20th century and has since been extensively studied, though its exact underlying cause remains incompletely understood. The term enostosis technically refers to a benign bony growth arising within the medullary cavity of a bone, while panosteitis denotes inflammation involving all layers of the bone. Both terms are used interchangeably in clinical practice to describe this specific developmental condition in dogs, though panosteitis is more commonly used in North America.
Enostosis typically affects dogs between five and eighteen months of age, though cases have been reported in dogs as young as two months and as old as five years. The condition has a notable predilection for male dogs, with males affected approximately four times more frequently than females, though the reason for this sex predisposition is not fully understood. German Shepherds are particularly overrepresented among affected dogs, and the condition appears to have a hereditary component in this breed.
The hallmark clinical feature of enostosis is its characteristic pattern of shifting leg lameness, where the affected limb changes from one leg to another over the course of days to weeks. This distinctive pattern, combined with the age and breed of the affected dog, often allows experienced clinicians to develop a strong clinical suspicion before diagnostic imaging is even performed. Despite the distress it causes to both dogs and their owners, the condition carries an excellent long-term prognosis, with virtually all affected dogs making a complete recovery.
