This condition develops over time through repeated hormonal exposure during estrous cycles and represents one of the most common reproductive disorders in unspayed female dogs. While CEH itself may not initially cause obvious symptoms, it creates conditions within the uterus that predispose to secondary bacterial infection and the development of pyometra, a potentially life-threatening uterine infection. The underlying mechanism of cystic endometrial hyperplasia involves the cumulative effects of progesterone on the uterine lining over successive reproductive cycles. During each estrous cycle, progesterone stimulates endometrial gland growth and secretory activity in preparation for potential pregnancy. In non-pregnant dogs, these changes regress, but over time and repeated cycles, the endometrium becomes increasingly abnormal. Glands become dilated and form cysts, the tissue thickens irregularly, and the normal architecture of the uterine lining becomes distorted. These changes create an environment favorable for bacterial colonization and infection.
The significance of CEH lies primarily in its relationship to pyometra, a serious uterine infection that can develop when bacteria ascend into the abnormal uterine environment. The cystic, thickened endometrium provides an ideal growth medium for bacteria, and the progesterone-dominated hormonal environment suppresses normal immune defenses within the uterus. Together, these factors can lead to rapid bacterial proliferation and pus accumulation within the uterine lumen. Pyometra is a medical emergency that requires prompt treatment, typically surgical removal of the uterus, to prevent life-threatening sepsis.
Treatment of cystic endometrial hyperplasia, particularly when complicated by pyometra, most commonly involves ovariohysterectomy, the complete surgical removal of the uterus and ovaries. This surgery eliminates both the diseased tissue and the hormonal stimulation that caused the condition. For valuable breeding dogs, medical management may be attempted in uncomplicated cases, though success rates vary and recurrence is common. Prevention through elective spaying of dogs not intended for breeding remains the most effective strategy. Any intact female dog showing signs of reproductive abnormalities should receive prompt veterinary evaluation to assess for CEH and related conditions.
