Cholelithiasis is the medical term for gallstones, which are solid concretions that form within the gallbladder or bile ducts of dogs. These stones develop when components of bile, including cholesterol, calcium salts, bilirubin, and other compounds, precipitate out of solution and aggregate into solid masses. Gallstones in dogs vary considerably in size, number, and composition, ranging from tiny sand-like particles to large stones several centimeters in diameter. While some dogs with gallstones remain asymptomatic and the stones are discovered incidentally during imaging for other purposes, others develop significant clinical signs when stones cause obstruction, inflammation, or other complications within the biliary system.
The development of gallstones in dogs occurs through a complex process involving alterations in bile composition, gallbladder motility, and local factors within the biliary system. Bile normally remains a balanced solution of bile acids, cholesterol, phospholipids, and other components. When this balance is disrupted, certain components may supersaturate the bile and begin to precipitate, forming crystals that eventually grow into stones. Reduced gallbladder contractility allows bile to remain stagnant within the gallbladder, concentrating and providing time for stone formation. Inflammation of the gallbladder wall may promote crystal nucleation and stone growth. Certain metabolic conditions, dietary factors, and breed predispositions influence the likelihood of stone formation.
The impact of cholelithiasis on canine health depends largely on whether the stones cause clinical signs or remain silent. Many dogs with gallstones are completely asymptomatic, with the stones discovered during abdominal imaging performed for unrelated reasons. These dogs may never develop problems from their gallstones. However, when gallstones move from the gallbladder into the bile ducts, they can cause obstruction of bile flow, leading to jaundice, abdominal pain, and serious illness. Stones within the gallbladder can contribute to chronic inflammation and cholecystitis. Large stones or multiple stones may impair normal gallbladder function. The presence of gallstones increases risk for other biliary complications including infection and rupture.
Cholelithiasis is treatable, though the approach depends on whether the stones are causing clinical problems. Asymptomatic gallstones discovered incidentally may be monitored without immediate intervention, though the decision requires careful consideration of potential risks. Dogs with symptomatic gallstones typically require surgical treatment through cholecystectomy, removal of the gallbladder, which eliminates both the stones and the site of stone formation. Medical management may help control secondary inflammation and infection, though it does not dissolve existing stones as effectively as in humans. Early detection and appropriate management help prevent serious complications.
