Canine anal furunculosis, more commonly referred to as perianal fistulas, is a chronic and debilitating inflammatory condition characterized by the development of deep, ulcerative draining tracts in the tissues surrounding the anus. The disease process involves progressive destruction of the perianal skin and underlying tissue, creating painful sinuses and fistulous tracts that communicate between the skin surface and deeper structures including the anal sacs, rectal wall, and surrounding musculature.
The condition was historically considered a surgical disease, with treatment focused on excision of affected tissue. However, advances in understanding the underlying immunopathology have shifted the treatment paradigm significantly over the past two decades. Current evidence strongly supports an immune-mediated basis for the disease, with aberrant T-cell responses driving the chronic inflammatory destruction of perianal tissues. This understanding has led to the adoption of immunosuppressive medical therapy as the primary treatment approach.
Anal furunculosis typically affects middle-aged to older dogs, with most cases diagnosed between five and eight years of age. The condition rarely occurs in dogs under two years old. Males and females are affected with roughly equal frequency, though some studies have reported a slight male predominance. The disease is progressive without treatment, and affected dogs experience significant pain and reduced quality of life as lesions expand and deepen over time.
The condition has a well-documented breed predisposition, with German Shepherd Dogs dramatically overrepresented in virtually every published case series. The anatomic conformation of the German Shepherd's tail set, combined with potential breed-specific immune characteristics, is believed to contribute to this susceptibility. However, any breed can be affected, and clinicians should not exclude the diagnosis based solely on breed.
