Bloat in dogs refers to a spectrum of conditions involving abnormal distension of the stomach with gas, fluid, or food. The term encompasses two related but distinct clinical presentations: gastric dilatation, in which the stomach expands but remains in its normal anatomical position, and gastric dilatation-volvulus, in which the distended stomach rotates on its axis, trapping its contents and cutting off blood supply to the stomach and surrounding organs. Gastric dilatation-volvulus, commonly abbreviated as GDV, is one of the most acute, life-threatening emergencies encountered in veterinary medicine.
In a normal dog, the stomach is anchored in the upper abdomen by ligamentous attachments and is free to expand moderately as it fills with food, water, and gas during digestion. In gastric dilatation, the stomach becomes excessively distended, typically with gas, which causes abdominal discomfort and can impede normal breathing by pressing on the diaphragm. While uncomfortable and potentially serious, simple gastric dilatation without volvulus can often be resolved with relatively straightforward medical intervention.
Gastric dilatation-volvulus occurs when the distended stomach rotates, most commonly in a clockwise direction when viewed from the surgeon's perspective, twisting anywhere from 90 to 360 degrees or more. This rotation occludes the gastric inlet at the esophagus and the outlet at the pylorus, preventing the escape of gas and fluid from the stomach. More critically, the rotation compresses the major blood vessels in the abdomen, including the caudal vena cava and portal vein, severely compromising venous return to the heart and triggering a cascade of cardiovascular, metabolic, and organ-level complications.
The mortality rate for GDV is significant even with prompt treatment, ranging from approximately 10 to 30 percent depending on the severity of the case, the speed of intervention, and the presence of complications such as gastric necrosis or cardiac arrhythmias. Without treatment, GDV is invariably fatal. The condition can progress from initial signs to life-threatening cardiovascular collapse in a matter of hours, making rapid recognition and immediate veterinary intervention absolutely critical for survival.
