Biliary mucocele is an increasingly recognized and potentially life-threatening condition of the gallbladder in dogs characterized by the abnormal accumulation of thick, gelatinous mucus within the gallbladder. This condition occurs when the gallbladder lining produces excessive amounts of mucus that gradually fills and distends the organ, creating a firm, immobile mass of inspissated bile and mucus. The accumulated material takes on a characteristic appearance resembling a kiwi fruit on ultrasound imaging, with a stellate or starfish-like pattern radiating from the center. Biliary mucocele has become one of the most commonly diagnosed gallbladder conditions in dogs over recent decades and represents a significant cause of biliary tract disease requiring surgical intervention.
The gallbladder normally functions to store and concentrate bile produced by the liver before releasing it into the small intestine to aid in fat digestion. In dogs with biliary mucocele, the normal function of the gallbladder becomes progressively impaired as mucus accumulates and bile becomes trapped within the thickened, gel-like contents. The gallbladder wall may become stretched, inflamed, and progressively weakened as the condition advances. In severe cases, the distended gallbladder can rupture, spilling its contents into the abdominal cavity and causing life-threatening bile peritonitis. The condition may also cause obstruction of bile flow, leading to jaundice and liver damage.
The impact of biliary mucocele on affected dogs ranges from subclinical disease detected incidentally to acute, life-threatening illness. Some dogs with early or mild mucocele formation may show no clinical signs, with the condition discovered during routine abdominal ultrasound for other reasons. However, as the condition progresses, dogs typically develop vomiting, decreased appetite, abdominal pain, lethargy, and potentially jaundice. When gallbladder rupture occurs, dogs may present in acute crisis with severe abdominal pain, collapse, and signs of sepsis. The variable presentation means some dogs receive treatment before serious complications develop while others present only when the condition becomes critical.
Biliary mucocele is primarily treated through surgical removal of the gallbladder, called cholecystectomy. Early surgical intervention before rupture or severe complications develop offers the best prognosis, with survival rates generally good when surgery is performed electively in stable patients. However, emergency surgery following gallbladder rupture carries significantly higher mortality rates. This emphasizes the importance of monitoring dogs at higher risk and pursuing prompt veterinary evaluation when symptoms develop. Medical management alone is not effective for resolving mucocele, though supportive care and addressing underlying conditions may help stabilize patients before surgery.
