Biceps tenosynovitis is an inflammatory and degenerative condition affecting the biceps brachii tendon and its surrounding synovial sheath where they pass through the bicipital groove at the front of the shoulder joint in dogs. The biceps tendon originates from the supraglenoid tubercle within the shoulder joint, travels through a specialized groove in the humerus called the bicipital groove, and inserts on the radius and ulna of the forearm. This tendon plays a crucial role in shoulder stability and forelimb function, and inflammation or degeneration of this structure causes significant lameness and discomfort. Biceps tenosynovitis is recognized as one of the most common causes of chronic forelimb lameness in medium to large breed dogs.
The biceps tendon is surrounded by a sheath of synovial membrane that provides lubrication and allows smooth gliding of the tendon within its groove during shoulder movement. The bicipital groove itself is formed by the humerus and is bridged by the transverse humeral ligament, creating a fibro-osseous tunnel through which the tendon passes. When this anatomical region becomes inflamed, the normal smooth gliding mechanism is disrupted, causing pain with shoulder movement. Over time, chronic inflammation leads to progressive degeneration of the tendon fibers, adhesion formation between the tendon and its sheath, and structural changes that perpetuate dysfunction.
Biceps tenosynovitis typically develops gradually as a result of repetitive strain, chronic microtrauma, or secondary to other shoulder pathology. Dogs engaged in athletic activities, working roles, or simply those with conformational factors predisposing to shoulder stress are most commonly affected. The condition may occur in isolation or in conjunction with other shoulder problems such as shoulder instability, supraspinatus tendinopathy, or osteochondritis dissecans. Understanding whether biceps tenosynovitis is a primary or secondary condition is important for developing an effective treatment plan, as addressing only the biceps while ignoring other shoulder pathology leads to treatment failure.
The prognosis for biceps tenosynovitis depends on the severity of tendon damage, presence of concurrent shoulder problems, and the treatment approach selected. Many dogs respond well to conservative management including rest, anti-inflammatory medications, and physical therapy. Dogs that fail to improve with conservative care or those with significant structural damage may require surgical intervention. With appropriate treatment, most dogs achieve significant improvement in comfort and function, though some may require activity modifications long-term. Early diagnosis and treatment generally yield better outcomes than delayed intervention after chronic degeneration has occurred.
