Aseptic necrosis of the femoral head, most commonly referred to as Legg-Calve-Perthes disease in veterinary medicine, is a degenerative orthopedic condition in which the blood supply to the femoral head is disrupted. The femoral head is the ball-shaped top of the thighbone that fits into the acetabulum of the pelvis to form the hip joint. When adequate blood flow to this region is compromised, the bone tissue begins to die in a process known as avascular necrosis. Over time, the weakened femoral head deteriorates, collapses, and becomes irregularly shaped, leading to severe pain, inflammation, and loss of normal hip joint function.
This condition predominantly affects small and toy breed dogs, typically presenting between four and twelve months of age during the period of rapid skeletal growth. Although the disease can occur in medium and larger breeds on rare occasions, it is overwhelmingly a condition of miniature breeds. Most cases are unilateral, affecting only one hip, though bilateral involvement has been documented in approximately ten to fifteen percent of cases. The onset of clinical signs is usually gradual, with progressive lameness developing over several weeks to months.
The precise etiology of aseptic necrosis of the femoral head remains incompletely understood, but it is widely accepted that a disruption in the vascular supply to the developing femoral epiphysis is the central pathological event. Without sufficient blood flow, the bone cells within the femoral head undergo ischemic necrosis. The resulting structural weakening makes the femoral head vulnerable to microfractures and eventual collapse under normal weight-bearing forces. This collapse leads to incongruity of the joint surfaces, cartilage damage, secondary osteoarthritis, and chronic pain.
Research into this condition has been ongoing for over a century, with parallels drawn to the human pediatric condition of the same name first described independently by Legg, Calve, and Perthes in 1910. In dogs, the condition was first formally recognized in veterinary literature in the mid-twentieth century, and since then significant advances have been made in understanding its pathophysiology, genetics, and optimal treatment approaches. Despite these advances, early recognition and timely surgical intervention remain critical to achieving the best outcomes for affected dogs.
