Cervical spine problems may cause head positioning changes that resemble vestibular disease. Severe dental disease causing pain may result in head tilting.

Potential complications of vestibular disease extend beyond the immediate balance and neurological issues. Secondary injuries from falls or rolling can include skin wounds, eye injuries from hitting cage surfaces, or even fractures in severe cases. GI stasis from not eating is a common and dangerous complication requiring aggressive management. Pressure sores may develop in guinea pigs who cannot move normally and remain in one position. Aspiration pneumonia can occur if guinea pigs with impaired coordination inhale food or water. Depression and reduced quality of life from inability to engage in normal activities affects some guinea pigs significantly. Chronic head tilt, while not dangerous in itself, may persist as a permanent reminder of the condition even in otherwise recovered guinea pigs.
