Excellent husbandry standards should be maintained throughout the breeding facility. All guinea pigs should receive proper nutrition including adequate vitamin C supplementation. Quarantine and health screening protocols for new animals help prevent introducing infectious diseases to the colony. Long-haired breeds in the breeding program should receive regular grooming attention to their ear areas.

Otitis interna is closely related to other ear conditions, as it typically develops as a progression of disease from the outer and middle ear. Otitis externa (outer ear infection) and otitis media (middle ear infection) commonly precede otitis interna, with infection progressively spreading from superficial to deeper structures. Guinea pigs presenting with otitis interna should always be assumed to have concurrent middle and outer ear disease that also requires treatment. Upper respiratory infections are frequently associated, as the bacteria causing respiratory disease, particularly Bordetella bronchiseptica, can spread to the ears through the Eustachian tubes. Vestibular syndrome or head tilt is a common clinical presentation of otitis interna, with the inner ear infection being the underlying cause of the vestibular signs.

Several conditions can present with symptoms similar to otitis interna and must be differentiated through careful veterinary evaluation. Central vestibular disease, caused by problems in the brain rather than the ear, can cause similar head tilt and balance problems but has a different prognosis and treatment approach. Brain tumors, particularly pituitary adenomas, can cause progressive neurological signs. Stroke can cause sudden onset vestibular signs, though this is relatively rare in guinea pigs. Encephalitozoon cuniculi, a microsporidian parasite more commonly seen in rabbits, occasionally affects guinea pigs and can cause vestibular symptoms. Head trauma and certain toxicities can also affect balance centers.

Potential complications of otitis interna include several secondary problems. The most serious complication is intracranial extension, where the infection spreads from the inner ear into the brain or meninges, causing encephalitis or meningitis, which is often fatal. Facial nerve paralysis can result from involvement of the facial nerve as it passes through the ear structures, causing drooping of the lip or eyelid. Permanent hearing loss can result from damage to the cochlea. Corneal ulcers can develop if the eye on the affected side does not close properly or is traumatized during rolling episodes. GI stasis can develop secondary to decreased food intake. Weight loss and nutritional deficiencies are common during the acute illness phase. Long-term antibiotic therapy can disrupt gut flora, potentially causing dysbiosis, so probiotic supplementation is typically recommended.
