Meningitis and encephalitis produce neurological symptoms that may include vestibular dysfunction along with more severe signs. Stroke can cause sudden-onset head tilt, circling, and loss of balance. Brain tumors affecting the brainstem balance centers produce progressive vestibular signs. Encephalitozoon cuniculi, a parasitic infection, occasionally causes vestibular symptoms in guinea pigs. Toxicosis from certain plants or medications can cause neurological abnormalities. Head trauma may damage vestibular structures directly.

Potential complications of middle ear infection include several serious conditions that can develop during or after the initial infection. Spread of infection to the inner ear structures can cause permanent vestibular damage and hearing loss. Extension to the meninges causes meningitis, a life-threatening complication. Brain abscess formation may occur with severe or chronic infections. Facial nerve paralysis develops when inflammation affects the nerve as it passes through the middle ear. Chronic otitis media with ongoing ear discharge and discomfort may develop despite treatment. Recurrent infections may occur, particularly if underlying predisposing factors persist. Secondary complications from vestibular dysfunction include aspiration pneumonia from difficulty swallowing, dehydration from inability to access water, malnutrition from reduced food intake, and pressure sores from limited mobility.
