Frostbite is direct cold injury to tissue that can occur alongside hypothermia, particularly affecting the ears, feet, nose, and other extremities with thin skin and limited blood flow. When body temperature drops, blood flow is redirected away from extremities to preserve core temperature, leaving these areas vulnerable to freezing. Frostbitten tissue may initially appear pale, cold, and hard, later becoming red, swollen, and painful as it rewarms. Severely frostbitten tissue may die and slough off over the following days to weeks. Treatment involves gradual rewarming, pain management, and wound care. The extent of tissue loss may not be apparent for several days after the initial cold exposure.

Gastrointestinal stasis frequently develops in hypothermic guinea pigs because cold slows digestive function. As body temperature drops, gut motility decreases, and the digestive system may essentially stop functioning. Once stasis develops, it can become a life-threatening condition in its own right, with gas accumulation, bacterial overgrowth, and toxin absorption. Even after body temperature is restored, the digestive system may remain sluggish, requiring prokinetic medications, fiber supplementation, and assisted feeding to restart normal function. Prevention and treatment of GI stasis is a critical component of hypothermia management.

Respiratory infections commonly develop in the days following cold exposure as a result of immune suppression during hypothermia. Upper respiratory infections in guinea pigs cause sneezing, nasal discharge, labored breathing, and loss of appetite. These infections can rapidly progress to pneumonia, which is life-threatening in guinea pigs. Guinea pig-safe antibiotics are required for treatment, with penicillin-type antibiotics and cephalosporins being contraindicated due to their fatal effects on guinea pig gut flora. Guinea pigs that have experienced hypothermia should be monitored closely for respiratory symptoms for at least one to two weeks after the cold exposure. Shock, which shares many symptoms with hypothermia including cold extremities, weak pulse, and altered consciousness, can develop from various causes and may lead to secondary hypothermia as the body loses its ability to regulate temperature during circulatory failure.
