The delayed onset makes ongoing monitoring essential even when a guinea pig appears to have recovered from the initial shock. Symptoms include increased breathing rate and effort, open-mouth breathing, pink frothy discharge from nose or mouth, and cyanosis. Without treatment, pulmonary edema is fatal. Other causes of pulmonary edema in guinea pigs include heart failure, pneumonia, and toxin exposure.

Cardiac arrhythmias are potentially fatal complications of electrical injury that may occur immediately or develop over the hours following exposure. Electrical current passing through the heart disrupts normal electrical conduction pathways, leading to abnormal heart rhythms. Some arrhythmias cause no obvious symptoms but may lead to sudden death. Others produce weakness, collapse, or observable irregularities in heart rhythm. Cardiac disease from other causes including cardiomyopathy can produce similar arrhythmias and may be considered in differential diagnosis if electrical exposure is not witnessed. Guinea pigs with underlying heart conditions may be more vulnerable to fatal arrhythmias from electrical shock.

Oral burns from electrical injury share characteristics with other types of oral trauma and infection in guinea pigs. Dental abscesses can cause swelling, pain, and tissue damage in the mouth that might initially resemble electrical burns. Oral injuries from sharp food items, cage components, or fighting with cage mates may appear similar to electrical burns on initial examination. Oral foreign bodies can cause drooling and eating difficulties similar to burn symptoms. A history of access to electrical cords and the characteristic appearance and location of electrical burns typically allow differentiation. However, in cases where electrical exposure was not witnessed, these other causes of oral pathology must be considered. Respiratory infections with symptoms including labored breathing, nasal discharge, and lethargy may initially resemble the respiratory complications of electrical injury, particularly if the electrical incident was not observed.
