Rectal prolapse causes visible tissue protrusion from the anus that may initially be confused with severe impaction, but examination reveals actual intestinal tissue rather than the perineal sac contents. Tumors in the perineal region create masses that may resemble impaction but have different characteristics on palpation. Severe constipation causes straining and abdominal discomfort but does not produce the characteristic perineal mass of impaction. Perianal abscesses present with swelling in similar locations but are typically focal, fluctuant, and may show signs of active infection. Parasitic infection occasionally affects the perineal area and may cause similar hygiene challenges.

Potential complications of impaction itself include several related problems. Secondary skin infection can develop when prolonged contact with accumulated material irritates and damages the surrounding skin, creating opportunity for bacterial invasion. Fly strike represents a serious risk during warm weather if the soiled area attracts flies that lay eggs, with resulting maggot infestation constituting a medical emergency. Fecal impaction of the rectum may occur if the perineal sac mass becomes large enough to physically obstruct normal defecation. Chronic discomfort from neglected impaction can contribute to reduced appetite and activity, potentially leading to secondary GI stasis which is life-threatening in guinea pigs. Social stress may develop if cage mates react negatively to the altered scent of an affected guinea pig.
