Hair rings, where shed hair accumulates around the penis creating a constrictive band, cause swelling and inability to retract the penis similar to severe balanoposthitis but require physical removal of the hair rather than antibiotic therapy. Boar glue, the accumulation of normal sebaceous secretions in the perianal area, may extend to involve the genital region and contributes to hygiene challenges. Penile or preputial tumors can cause swelling and discharge that mimics infection. Trauma to the genital area, whether from cage mates, environmental hazards, or mating activity, produces inflammation that may initially appear similar to infection.

Potential complications of balanoposthitis extend beyond the immediate infection. Scarring of the prepuce or penis following severe or recurrent infections may permanently alter anatomy and function. Phimosis, narrowing of the preputial opening due to scarring, can trap smegma and debris, predisposing to future infections. Chronic pain or sensitivity may persist even after infection resolves, affecting quality of life. Fertility may be impaired in breeding males if infection has affected the testes or caused significant scarring. Secondary gut issues including GI stasis may develop if pain and stress reduce appetite and food intake, representing a potentially life-threatening complication that requires concurrent management.
