The underlying cause of von Willebrand disease is a genetic mutation affecting the production or function of von Willebrand factor. This protein is synthesized by endothelial cells lining blood vessels and by megakaryocytes in the bone marrow. In affected individuals, mutations in the gene encoding von Willebrand factor result in either reduced quantity or abnormal function of the protein. Von Willebrand disease is classified into different types based on whether the protein is quantitatively reduced or qualitatively abnormal. In dogs and humans, three main types are recognized, though the specific types that occur in the rare feline cases have not been fully characterized due to the limited number of diagnosed cats.
The impact of von Willebrand disease on affected cats can range from minimal to life-threatening depending on the severity of the factor deficiency. Cats with mild disease may show little clinical evidence of bleeding problems and might only be diagnosed when excessive bleeding occurs during surgery or after significant trauma. More severely affected individuals may experience spontaneous bleeding episodes, prolonged bleeding from minor wounds, and potentially dangerous hemorrhage. The condition affects the primary hemostatic process, which is the initial platelet plug formation at sites of blood vessel injury, resulting in characteristic mucosal bleeding rather than the deep tissue hemorrhage seen with coagulation factor deficiencies.
Von Willebrand disease in cats is manageable with appropriate care, though there is no cure for this genetic condition. Treatment focuses on preventing bleeding episodes through careful management and providing support during times of increased bleeding risk such as surgery or trauma. Blood transfusions containing von Willebrand factor can be life-saving during active bleeding crises. The extreme rarity of this condition in cats means that veterinary experience with feline cases is limited, and management approaches are often extrapolated from the more extensive experience with canine von Willebrand disease. Early recognition and appropriate preventive measures can help affected cats live relatively normal lives.
