Pyometra is a serious and potentially life-threatening bacterial infection of the uterus that occurs in intact female cats. The condition is characterized by the accumulation of pus within the uterine cavity, resulting from bacterial colonization of the hormonally-primed uterine lining. Pyometra represents a veterinary emergency that requires prompt diagnosis and treatment to prevent life-threatening complications including sepsis, endotoxemia, and death. While pyometra is somewhat less common in cats than in dogs due to differences in their reproductive physiology, it remains a significant health risk for any unspayed female cat and is one of the most compelling reasons veterinarians recommend spaying cats not intended for breeding.
The development of pyometra is intimately connected to the hormonal changes of the feline reproductive cycle. Following estrus and ovulation, the hormone progesterone dominates the reproductive tract, causing changes in the uterine lining that prepare it for potential pregnancy. However, these same progesterone-induced changes also create an environment that is susceptible to bacterial infection. The cervix relaxes during estrus, allowing bacteria from the vaginal tract to ascend into the uterus. If these bacteria colonize the progesterone-stimulated uterine lining, infection develops and pus accumulates. The condition typically develops within weeks to months following a heat cycle, with middle-aged to older cats being most commonly affected after experiencing many reproductive cycles.
The impact of pyometra on affected cats is severe and multisystemic. As the infection progresses, bacterial toxins are absorbed into the bloodstream, causing systemic inflammatory response syndrome and potentially septic shock. The enlarged, pus-filled uterus can cause abdominal discomfort and distension. Toxin effects on the kidneys may cause acute kidney injury, a common and serious complication of pyometra. Affected cats typically become progressively ill, showing signs including lethargy, loss of appetite, vomiting, increased thirst and urination, and fever, though some cats with closed-cervix pyometra may not display all classic signs until they are critically ill. Without treatment, pyometra is invariably fatal.
Treatment for pyometra is most commonly surgical, involving emergency ovariohysterectomy to remove the infected uterus and ovaries. This surgery is more complex and carries higher risk than elective spaying due to the patient's compromised condition, the fragile infected tissues, and the need for intensive perioperative care. With appropriate surgical treatment and supportive care, most cats can recover fully from pyometra. Medical management is occasionally attempted in valuable breeding cats where fertility preservation is desired, but this approach carries significant risks and has lower success rates than surgery. The critical importance of early recognition and treatment cannot be overstated, as delays allow the infection to progress and complications to develop.
