Panophthalmitis is a severe and potentially life-threatening ocular condition characterized by purulent inflammation affecting all structures of the eye and extending to involve the surrounding orbital tissues. This devastating infection represents one of the most serious emergencies in veterinary ophthalmology, requiring immediate and aggressive intervention to save the cat's life, as vision is typically already irretrievably lost by the time panophthalmitis develops. The condition progresses rapidly from initial infection through intraocular inflammation to involve the entire globe and periocular structures, creating a source of systemic sepsis that can threaten the cat's survival.
Panophthalmitis typically develops as a progression from endophthalmitis, which is infection confined within the eye, when the infectious process breaks through the outer coats of the eye to involve orbital tissues. The most common route of infection is through penetrating injuries to the eye, which introduce bacteria or fungi directly into intraocular structures. Other causes include extension of severe corneal infections, surgical complications, and hematogenous spread of infection from other body sites. Once established, the infection creates a closed space abscess filled with purulent material that progressively destroys all ocular structures.
The impact of panophthalmitis on the affected cat is profound and multifaceted. Locally, the infection causes complete destruction of the eye with total and permanent vision loss in the affected globe. Severe pain accompanies the intense inflammatory response, significantly impacting the cat's comfort and wellbeing. Systemically, the infected eye serves as a reservoir for bacteria that can spread throughout the body, potentially causing septicemia, organ failure, and death. Cats with panophthalmitis are often systemically ill with fever, lethargy, and decreased appetite in addition to their obvious ocular disease.
Treatment of panophthalmitis nearly always requires surgical removal of the affected eye, as the globe is beyond salvage and poses an ongoing threat to the cat's life while it remains. Aggressive systemic antibiotic therapy is essential to combat spreading infection. With prompt and appropriate treatment, including enucleation and antimicrobial therapy, most cats survive and recover well. The prognosis for life is generally good if treatment is initiated before overwhelming sepsis develops. However, delay in treatment can prove fatal, making immediate veterinary evaluation essential when panophthalmitis is suspected.
