Systemic mast cell tumor, also known as visceral mast cell tumor or disseminated mast cell disease, is a form of cancer in cats that affects internal organs rather than the skin. Mast cells are immune cells normally found throughout the body that play roles in allergic reactions and inflammation. When mast cells become cancerous and proliferate in internal organs, particularly the spleen, liver, bone marrow, and intestinal tract, the result is systemic mast cell disease. This is the second most common form of mast cell tumor in cats after cutaneous mast cell tumors, representing a significant proportion of feline hematopoietic cancers.
The development of systemic mast cell disease involves malignant transformation of mast cells within internal organs. The spleen is the most commonly affected organ, followed by the intestinal tract, liver, and bone marrow. The relationship between splenic and intestinal forms is not entirely clear, as they may represent distinct disease entities or different presentations of the same condition. Splenic mast cell tumors may remain relatively localized or spread to involve other organs, while intestinal mast cell disease often presents with multifocal involvement. Bone marrow infiltration, known as mastocytemia or mast cell leukemia, represents an advanced and more serious stage of disease.
The impact of systemic mast cell disease on affected cats depends on the organs involved and the extent of disease spread. Splenic involvement may cause splenomegaly, which can lead to abdominal distension and, if the spleen ruptures, life-threatening internal bleeding. Intestinal mast cell tumors cause gastrointestinal symptoms including vomiting, diarrhea, and weight loss. The release of histamine and other mediators from mast cells can cause systemic effects including gastric ulceration, cardiovascular changes, and allergic-type reactions. Advanced disease with bone marrow involvement affects blood cell production and can cause anemia and other cytopenias.
The prognosis and treatment of systemic mast cell tumors in cats varies depending on the specific presentation. Splenic mast cell tumors are often successfully managed with splenectomy alone, with many cats achieving long survival times. Intestinal mast cell tumors have a more variable prognosis depending on the extent and grade of disease. Disseminated disease involving multiple organs or bone marrow carries a more guarded prognosis but may still respond to treatment. Chemotherapy is used in cases where surgery alone is insufficient. Working with a veterinary oncologist allows for accurate staging, appropriate treatment planning, and the best possible outcomes for affected cats.
