Leishmaniasis is a parasitic disease caused by protozoan organisms of the genus Leishmania, transmitted primarily through the bite of infected sandflies. While this condition is relatively common in dogs in endemic regions, it remains rare in cats, though cases have been increasingly recognized in recent years. The disease occurs predominantly in Mediterranean countries, South America, the Middle East, and parts of Asia and Africa where sandfly vectors are present. Cats appear to have some natural resistance to Leishmania infection compared to dogs, but they can still develop clinical disease, particularly when their immune system is compromised by concurrent conditions such as feline immunodeficiency virus or feline leukemia virus.
The parasite enters the cat's body when an infected female sandfly takes a blood meal, injecting Leishmania promastigotes into the skin. These organisms are then engulfed by macrophages, where they transform into amastigotes and begin to multiply. The parasites can spread throughout the body via the lymphatic and circulatory systems, potentially affecting multiple organ systems including the skin, lymph nodes, spleen, liver, bone marrow, and kidneys. The incubation period can range from months to years, making the connection between sandfly exposure and disease development challenging to establish.
Leishmaniasis significantly impacts affected cats' health and quality of life, causing a range of clinical manifestations from mild skin lesions to severe systemic disease. The cutaneous form primarily affects the skin, causing nodules, ulcers, and scaling, while the visceral form involves internal organs and can lead to weight loss, lethargy, and organ dysfunction. Without appropriate treatment, the disease can progressively worsen and may become life-threatening, particularly when vital organs such as the kidneys are affected.
Treatment for feline leishmaniasis is available and can be effective in managing clinical signs, though complete elimination of the parasite is rarely achieved. Early detection improves treatment outcomes significantly, as cats diagnosed before severe organ damage occurs tend to respond better to therapy. Lifelong monitoring and sometimes ongoing treatment may be necessary to maintain remission. Veterinary care is essential for proper diagnosis and management, as the disease requires specific antiprotozoal medications and careful monitoring of organ function throughout treatment.
