Hookworms are blood-feeding parasitic nematodes that infect the small intestine of cats, causing disease ranging from mild gastrointestinal disturbance to severe, life-threatening anemia depending on the worm burden and the cat's age and health status. The two primary species affecting cats are Ancylostoma tubaeforme, the most common feline hookworm worldwide, and Ancylostoma braziliense, prevalent in tropical and subtropical regions. These small but voracious parasites attach to the intestinal lining using their characteristic hooked mouthparts and feed on blood, with each adult worm capable of consuming a measurable amount of blood daily. In heavy infections, particularly in young kittens, the cumulative blood loss can rapidly lead to severe anemia and potentially death if untreated.
Hookworm transmission occurs through multiple routes, making infection particularly common in cats with outdoor access. The most significant route is percutaneous penetration, where infective larvae present in contaminated soil actively penetrate through the cat's skin, typically through the paw pads. Oral ingestion of larvae from contaminated environments or during grooming of contaminated fur represents another important transmission route. Cats can also become infected by consuming prey animals harboring larvae in their tissues. Unlike in dogs, transmammary transmission (through mother's milk) is not a significant route in cats, though prenatal transmission may occur. Once inside the host, larvae migrate through tissues before reaching the small intestine where they mature into blood-feeding adults.
The impact of hookworm infection on feline health depends heavily on the intensity of infection and the host's age and immune status. Adult cats with light infections often show minimal clinical signs, as their immune systems can regulate worm burdens and compensate for modest blood loss. However, young kittens are exquisitely vulnerable to hookworm disease because they have small total blood volumes, limited iron reserves, and immature immune systems. Heavy infections in kittens can cause profound anemia, weakness, failure to thrive, and death within days of clinical signs appearing. Chronic infections in cats of any age can lead to persistent weight loss, poor coat condition, and ongoing gastrointestinal problems that diminish quality of life.
Hookworm infection is readily treatable with commonly available anthelmintic medications, and most cats recover completely with appropriate therapy. However, the zoonotic potential of feline hookworms, particularly Ancylostoma braziliense, represents a significant public health concern. Hookworm larvae can penetrate human skin and cause cutaneous larva migrans, an intensely itchy, serpentine skin eruption resulting from larval migration through the superficial skin layers. This condition is most common in people who walk barefoot or sit on contaminated sandy beaches or soil in endemic areas. Prevention through regular deworming of cats and responsible fecal disposal, combined with avoiding barefoot contact with potentially contaminated soil, protects both feline and human health.
