Hiatal hernia in cats is a condition in which a portion of the stomach protrudes through the esophageal hiatus, the normal opening in the diaphragm through which the esophagus passes to connect with the stomach. Mild cases may cause only occasional regurgitation and minimal discomfort, while severe cases can lead to chronic esophagitis, aspiration pneumonia, and life-threatening respiratory compromise. Because the stomach's position may shift between herniated and normal positions, symptoms can wax and wane unpredictably. Progressive damage to the esophagus from chronic acid exposure can lead to stricture formation that further complicates eating and swallowing. The variable presentation of hiatal hernia often delays diagnosis as symptoms may be attributed to other, more common conditions.
Hiatal hernia in cats is treatable, with management approaches ranging from medical therapy for mild cases to surgical correction for more severe presentations. Medical management focuses on reducing gastric acid production and protecting the esophageal lining from reflux damage. Surgical repair involves repositioning the stomach below the diaphragm and reconstructing the esophageal hiatus to prevent recurrence. With appropriate treatment, many cats with hiatal hernia can achieve significant symptom improvement and maintain good quality of life.
